If you are homebound and need to register for a mobile vaccination appointment, the process is as follows:
• Call the vaccine appointment hotline at 833-308-1988 or sign up online at this survey: http://bit.ly/homevaccine.
• The survey can also be accessed from the vaccine.cookcountyil.gov site – there is a button on the top right that says “in home vaccination.”
• For this program, appointments are being reserved for individuals who are seniors or those with a disability who require in-home assistance, need adaptive and/or accessible transportation to leave home, and for whom leaving home is not an option because doing so requires considerable and taxing effort.
