* Hours of sites are subject to change. Visit https://cookcountyhealth.org/covid19-vaccine-sites/ for updated hours of operation.
** Cook County Health’s mobile vaccination site in Summit will continue to offer walk-in availability however it is generally limited to individuals in the surrounding communities: Summit, McCook, Hodgkins, Justice, Bridgeview and Chicago Ridge.
To register through Cook County, call 833-308-1988, or visit vaccine.cookcountyil.gov.
If you are disabled and need assistance registering for your vaccine, you can contact Progress Center for Independent Living at 708-388-5011.
If you are homebound and need to register for a mobile vaccination appointment the process is as follows:
• Call the vaccine appointment hotline at 833-308-1988 or sign up online at this survey: http://bit.ly/homevaccine.
• The survey can also be accessed from the www.vaccine.cookcountyil.gov site – there is a button on the top right that says “in home vaccination”
• Please remember, that for this program we are reserving these appointments for individuals who are seniors or persons with a disability who require in-home assistance, need adaptive and/or accessible transportation to leave home, and for whom leaving home is not an option because doing so requires considerable and taxing effort. The survey and the hotline allow us to screen for eligibility, and individuals not eligible will be directed to the hotline/general microsite to make regular appointments.
Vaccines are also available through pharmacies like Walgreens, CVS, and Jewel-Osco, and by contacting your physician.
